By focusing on nutrition, physical activity, sleep, stress management, and substance avoidance, these nurses empower individuals to make sustainable health choices. They assess needs, provide education, and develop personalized action plans to guide patients toward healthier habits. Working as part of a multidisciplinary team, Lifestyle Health Nurses utilize their clinical expertise and excellent communication skills to support long-term wellness goals.
Scope of Practice:
Assessment:
Evaluate a patient’s lifestyle—including diet, exercise, and sleep—to identify areas for improvement.
Health Education & Counseling:
Offer guidance on making healthier choices, such as creating meal plans or stress reduction strategies.
Goal Setting:
Work collaboratively with patients to establish achievable, actionable health goals.
Care Coordination:
Partner with other healthcare providers to ensure the safety and effectiveness of each patient’s lifestyle plan.
Chronic Disease Management:
Assist patients in managing conditions like type 2 diabetes and heart disease through lifestyle modifications.
Accountability & Support:
