A Lifestyle Health Nurse, also known as a Nurse Health Coach or Lifestyle Medicine Nurse, is a registered nurse dedicated to helping patients prevent, manage, and even reverse chronic diseases through lifestyle changes. By focusing on nutrition, physical activity, sleep, stress management, and substance avoidance, these nurses empower individuals to make sustainable health choices. They assess needs, provide education, and develop personalized action plans to guide patients toward healthier habits. Working as part of a multidisciplinary team, Lifestyle Health Nurses utilize their clinical expertise and excellent communication skills to support long-term wellness goals.


Scope of Practice:


Assessment:

Evaluate a patient’s lifestyle—including diet, exercise, and sleep—to identify areas for improvement.


Health Education & Counseling:

Offer guidance on making healthier choices, such as creating meal plans or stress reduction strategies.


Goal Setting:

Work collaboratively with patients to establish achievable, actionable health goals.


Care Coordination:

Partner with other healthcare providers to ensure the safety and effectiveness of each patient’s lifestyle plan.


Chronic Disease Management:

Assist patients in managing conditions like type 2 diabetes and heart disease through lifestyle modifications.


Accountability & Support:

Provide ongoing follow-up to monitor progress, offer encouragement, and maintain motivation.

  • Lifestyle Health Assessment

    Think of the Lifestyle Health Assessment as a routine check-up—like your annual dental cleaning, eye exam, or physical. Designed to be a proactive touchpoint, this service helps you stay on track with your health and wellness goals without the need for ongoing coaching, unless you choose to integrate it.


    During this 50-minute session, we’ll review your current lifestyle, habits, and progress. Prior to your appointment, you’ll complete a self-assessment to evaluate areas like nutrition, activity, sleep, and stress. I’ll review your responses and prepare a personalized report to discuss during our session.


    The goal is to create a consistent, sustainable routine of self-coaching and peer support. These assessments occur every 3 to 6 months, giving you regular opportunities to reflect, adjust, and reinforce positive habits.

  • Six Month Health Coaching Membership

    A six month membership includes six in person sessions and a weekly vitural check in via email. In person sessions can happen at the Decatur Wellness Collective, or off site at a location that works for you!

Libby Pasquariello

Libby Pasquariello, MSN/ED, RN, HWNC-BC, is a health coach, educator, and patient advocate.  A registered nurse for twenty years now, she spent the first ten of those working with adult clients in mental health care settings.  During this time, she experienced the beauty of nursing through the interpersonal connections made with clients and families, but she also recognized the limits of the health care system.  She returned to complete graduate education as a nurse educator and has spent the last decade teaching in higher education.  After earning certification as a health coach nurse, now she aims to connect with individuals and small groups seeking to improve their overall wellness and quality of life.  She believes that illness and wellness lie on a spectrum and absence of illness does not always mean wellness.  There is a gap between the care provided by the traditional health care system and people’s daily lifestyles and this is where health coaching with a nurse can provide a bridge. 


 The Quiet Cornerstones of Well-Being:  Libby believes stress relief and the power of rest are undervalued in today’s society, resulting in a culture that celebrates busyness over quality of life.  She believes finding joy and cultivating a sense of playful curiosity about life are tangible skills for anyone seeking to improve their overall wellbeing and quality of life.  And she believes YOU should be the one to define what wellness looks like for you.
